PLEASE NOTE:

Requests should be made as far in advance as possible.
Rental fees and contractual terms for off campus groups MUST be arranged with the Business Office.
You must enter a valid Warner e-mail address for this form to work.
Use of facilities must first be scheduled and approved through the C.E.O.'s office before an activity request can be processed.
Usage of audio and or visual equipment must be authorized through the Audio/Visual Department located in the LRC.

WHEN USING Warner University FACILITIES,
WE UNDERSTAND THAT ALL MAINTENANCE AND
CUSTODIAL LABOR IS INCLUDED IN THE FACILITY
RENTAL RATES.

CHARGES FOR SPECIAL SETUPS AFTER 5 P.M. MONDAY
THROUGH FRIDAY AND ON WEEKENDS WILL BE BILLED
BY THE BUSINESS OFFICE.

RENTAL FOR PIANOS, AS APPROVED BY THE MUSIC
DEPARTMENT CHAIRMAN, WILL ALSO BE BILLED BY
THE MUSIC DEPARTMENT.
